Difficult pain

The article presents an excellent yet brief revision on the current management of difficult pain.

But when refers to acupuncture, reveals the incipient level of information about the therapeutic properties of the neurostimulation / neuromodulation techniques used by the Contemporary Medical Acupuncture.

When describes acupuncture as related to oriental medicine, reducing its therapeutic effects to the release of endogenous analgesics in the spinal cord, do not consider the presently available different kinds of neural intervention, that produce local, segmental, supra-segmental, sensory, motor and autonomic changes in nerve functions.

Among these interventions, electro-neural-stimulation, needle manipulation and a specific use of local anesthetics, all gathered under an unique medical specialty.

O artigo apresenta uma ótima e sucinta revisão do manejo atual das dores de difícil tratamento.

Quando se refere à acupuntura, revela que a informação sobre as propriedades dos métodos de neuroestimulação / neuromodulação utilizados pela Acupuntura Médica Contemporânea ainda é incipiente.

Ao descrever a acupuntura como relacionada com a medicina oriental, restringindo os seus efeitos terapêuticos à liberação de analgésicos endógenos na medula espinal, e ao apresentar a eletro-neuro-estimulação transcutânea em separado, deixa de considerar os diversos tipos de intervenção disponíveis atualmente, e que produzem efeitos locais, medulares e supra-segmentares, motores, sensoriais e autonômicos.

A eficácia dessas intervenções, que incluem eletro-neuro-estimulação, um uso específico de infiltrações de anestésicos locais, além da manipulação de agulhas, está bem estabelecida, que poderiam estar agrupadas sob uma mesma especialidade.
